Defend. 3NT?! On the evolving bidding shown, I'd rather play 3D - which makes: losing two Clubs, one Diamond, and one Heart (unlikely East will double - unless he's playing the opponents rather than the cards). Even 3HX plays better than 3NTX.... [I would have been playing a bland 2C or 3C, making 4 on the bidding 1D - 1H - 2C - pass or 1D - 1H - 2C - 2H (ill-advised given the quality of North's Hearts) - 3C - pass.]

What do mean lead a small heart ? The seven ? How can partner ever know that you started with a doubleton?Lead the 8 and I can know to play the ace to drop the singleton king. Either way I'll defend. It looks like at least a one trick set.
